areas listed below, you will be expected to complete your midterm and final
exam(s) with you instructor(s) on the day, time and locations listed in the course
syllabus. There are a limited number of alternate exam dates available for students that
are not available to attend the exam on the date listed in the syllabus. If
your zip code does not match any of the areas listed below, you may have your
exam proctored by an appropriate proctor. It is each student’s responsibility to
return the completed proctor form to the Distance Learning Department by the end of the
first week of the course. Each student should also verify that the proctor meets the
requirements listed on the Proctor Agreement Form.
